New research confirms efficacy of transcranial magnetic stimulation for depression

In one of the first studies to look at transcranial magnetic stimulation (TMS) in real-world clinical practice settings, researchers at Butler Hospital, along with colleagues across the U.S., confirmed that TMS is an effective treatment for patients with depression who are unable to find symptom relief through antidepressant medications. The study findings are published online in the June 11, 2012 edition of Depression and Anxiety in the Wiley Online Library.

Previous analysis of the efficacy of TMS has been provided through more than 30 published trials, yielding generally consistent results supporting the use of TMS to treat depression when medications aren't sufficient. "Those previous studies were key in laying the groundwork for the FDA to approve the first device for delivery of TMS as a treatment for depression in 2008," said Linda Carpenter, MD, lead author of the report and chief of the Mood Disorders Program and the Neuromodulation Clinic at Butler Hospital. "Naturalistic studies like ours, which provide scrutiny of real-life patient outcomes when TMS therapy is given in actual clinical practice settings, are the next step in further understanding the effectiveness of TMS. They are also important for informing healthcare policy, particularly in an era when difficult decisions must be made about allocation of scarce resources."

Carpenter explains that naturalistic studies differ from controlled clinical trials because they permit the inclusion of subjects with a wider range of symptomatology and comorbidity, whereas controlled clinical trials typically have more rigid criteria for inclusion. "As a multisite study collecting naturalistic outcomes from patients in clinics in various regions in the U.S., we were also able to capture effects that might arise from introducing a novel psychiatric treatment modality like TMS in non-research settings," said Carpenter. In all, the study confirms how well TMS works in diverse settings where TMS is administered to a real-life population of patients with depression that have not found relief through many other available treatments.

The published report summarized data collected from 42 clinical TMS practice sites in the US, and included outcomes from 307 patients with Major Depressive Disorder (MDD) who had persistent symptoms despite the use of antidepressant medication. Change during TMS was assessed using both clinicians' ratings of overall depression severity and scores on patient self-report depression scales, which require the patient to rate the severity of each symptom on the same standardized scale at the end of each 2-week period. Rates for "response" and "remission" to TMS were calculated based on the same cut-off scores and conventions used for other clinical trials of antidepressant treatments. Fifty-eight percent positive response rate to TMS and 37 percent remission rate were observed.

"The patient outcomes we found in this study demonstrated a response rate similar to controlled clinical trial populations," said Dr. Carpenter, explaining that this new data validates TMS efficacy in treating depression for those who have failed to benefit from antidepressant medications. "Continued research and confirmation of the effectiveness of TMS is important for understanding its place in everyday psychiatric care and to support advocacy for insurance coverage of the treatment." Thanks in part to the advocacy efforts of Dr. Carpenter, TMS was recently approved for coverage by Medicare in New England, and it is also now covered by BCBSRI. "Next steps for TMS research involve enhancing our understanding of how to maintain positive response to TMS over time after the course of therapy ends and learning how to customize the treatment for patients using newer technologies, so TMS can help even more patients."

Aging AIDS epidemic raises new health questions

WASHINGTON (AP) ? AIDS is graying. By the end of the decade, the government estimates, more than half of Americans living with HIV will be over 50. Even in developing countries, more people with the AIDS virus are surviving to middle age and beyond.

That's good news ? but it's also a challenge. There's growing evidence that people who have spent decades battling the virus may be aging prematurely. At the International AIDS Conference this week, numerous studies are examining how heart disease, thinning bones and a list of other health problems typically seen in the senior years seem to hit many people with HIV when they're only in their 50s.

"I'm 54, but I feel older," said Carolyn Massey of Laurel, Md., who has lived with HIV for nearly 20 years.

"When I hear young people talk about, 'Well you get HIV and you take your drugs and you'll be all right' ? that's just not the truth," she said. "This is a lifelong thing we're talking about, and it unfolds every day on you."

The graying isn't just because people like Massey are surviving longer. Some of it comes from older adults being newly diagnosed, a trend U.S. health officials say is small but slowly growing. Yes, grandparents still have sex ? and that's an age group missed by all those hip safe-sex messages aimed at teens and 20-somethings.

"They let down their guard," is how Dr. Kevin Fenton of the Centers for Disease Control and Prevention puts it.

Already, a third of the nearly 1.2 million people living with HIV in the U.S. are over 50, and by 2020 half will be, Fenton said at one of numerous sessions on aging at the world's largest AIDS meeting.

People 50 or older accounted for 17 percent of new HIV diagnoses in 2009, according to the CDC's latest data. That's up from 13 percent in 2001.

There aren't as good counts in poor regions of the world, where access to life-saving medications came years later than in developed countries.

But even in hard-hit sub-Saharan Africa, home to most of world's HIV-infected population, studies suggest 3 million people living with HIV are 50-plus, said Dr. Joel Negin of the University of Sydney in Australia. By 2040, he said, that could reach 9 million.

There, challenges are different. Ruth Waryaro of Kenya, addressing the conference on her 65th birthday, said clinic workers hassle her when she goes to pick up her monthly supply of medication ? not believing a grandmother really needs it.

"If you're not strong enough, you just leave the medication and go home," said Waryaro, who raised four children of her own and now is raising four AIDS orphans. She also has diabetes and high blood pressure.

As Negin pointedly told the conference, "50 is not old." But for years, world health authorities didn't even measure HIV in people beyond age 49.

Today, people who are diagnosed and treated early can expect a near-normal life-span, Dr. Anthony Fauci, infectious disease chief at the National Institutes of Health, told The Associated Press.

The new focus is on what these pioneering survivors can expect as they reach their 50s, 60s and beyond. They're now getting chronic illnesses such as heart disease, diabetes, kidney disease and osteoporosis ? some of the common ailments when anyone gets old. But studies suggest people with HIV may be at higher risk for some of those illnesses, or get them earlier than usual.

"It's almost created a new subspecialty of medicine," Fauci said.

Perhaps the strongest evidence links HIV and an increased risk of heart disease. Some AIDS medications raise that risk. But in research published for the AIDS meeting, scientists at Massachusetts General Hospital uncovered another reason.

They scanned the arteries of people with and without HIV, and found the HIV patients had more inflammation inside their arteries, putting them at risk for the kind of clots that trigger heart attacks. That's even though the HIV patients had their virus well-controlled and weren't that old ? their average age was 52, the researchers reported in the Journal of the American Medical Association.

HIV triggers body-wide inflammation as a person's immune system tries to fight the virus, a process that persists and can quietly damage organs even with good medications, CDC's Fenton said.

HIV is not acting in a vacuum, said Dr. Amy Justice of Yale University, noting that people's histories of smoking, for example, also contribute to inflammation.

But she pointed to data from a Veterans Affairs study that said older people with HIV use more medications for other diseases than HIV-free patients the same age.

At the conference, some older people with HIV lined up to have their photographs made and their personal histories recorded, part of a Web project called "The Graying of AIDS."

It's a chance to be counted, and share knowledge.

"We're so concerned about the youth factor, we forget about the people who've brought us thus far," said Massey, the Maryland woman, who leads an HIV group called Older Women Embracing Life and works with churches to raise HIV awareness.

CDC's Fenton noted that those voices can help other older adults realize they're at risk, when they're getting back onto the dating scene after years of monogamous relationships. Older people don't use condoms as much as younger people.

"We still have this huge issue with stigma so thick you can cut it with a knife," says Massey, who also wants HIV testing to become a routine part of health check-ups. "We have to normalize the conversation."

TSX rallies to three-week high on euro zone hopes

TORONTO (Reuters) - Canadian stocks hit a three-week high on Friday, led by financial and energy shares, as risk trade rallied on renewed optimism that European policymakers were united in battling the euro zone's debt crisis that is sapping global economic growth.

German Chancellor Angela Merkel and French President Fran?ois Hollande said on Friday that they are 'deeply committed to the integrity of the euro zone' and 'are determined to do everything to protect the euro zone.' The joint statement echoed similar remarks by European Central Bank President Mario Draghi on Thursday.

Equities extended gains after Bloomberg News said Draghi will meet with Bundesbank President Jens Weidmann to discuss several measures, including bond purchases, to help the euro zone.

'Today they came out with a fairly unified statement about the kinds of things they'd like to do, including bond buying and interest rate cuts,' said Rick Meslin, head of Canadian equities at UBS Securities Canada.

Meslin added the comments likely encouraged some of the larger investment funds to get off the sidelines, providing 'a lot of ammunition to buy stocks.'

The ECB, which cut interest rates to a record low in early July, will meet again next Thursday to decide on interest rates and discuss policy measures. It has previously bought government bonds in the secondary market to push yields lower, and pressure is building on it to do more.

'If that's the case it will be removing the really bad outcome of a financial collapse,' said Carlos Leitao, chief economist at Laurentian Bank Securities.

The news lifted Canada's financial services sector, which rose 1.5 percent to lead the broader index higher. Gains were led by the country's top lenders, with Royal Bank of Canada up 1.7 percent at C$51.73, Toronto-Dominion Bank up 1.4 percent at C$79.48, and Bank of Nova Scotia rising 1.4 percent to C$52.02.

The heavyweight oil and gas group climbed 1.8 percent as U.S. crude rose for a fourth day on hopes for more stimulus from the U.S. Federal Reserve after data showed U.S. economic growth slowed in the second quarter as expected.

Progress Energy Resources Corp led energy gains, soaring nearly 14 percent to C$22.85 after the Canadian natural gas producer said Malaysia's state-owned oil company Petronas has agreed to raise its offer to buy the company by 8 percent.

The deal is the latest by state-owned Asian companies for companies in the Canadian energy patch. Earlier this week, Chinese state-owned oil company CNOOC Ltd's offered $15.1 billion for Canadian oil producer Nexen Inc in what would be China's largest foreign deal.

Also supporting the sector's rise was Talisman Energy, which jumped 5.7 percent to C$12.71, and Encana Corp, up 4.7 percent at C$21.30.

Paring gains was Cameco Corp, which tumbled 2.5 percent to 22.08 after the uranium producer reported an 85 percent drop in quarterly profit on Friday on lower sales volume, higher costs and a one-time charge for a canceled sales contract.

The Toronto Stock Exchange's S&P/TSX composite index closed up 126.61 points, or 1.1 percent, at 11,766.36. The index at one point touched 11,802.51?its highest level since July 5. It also ended up 1.2 percent for the week.

The influential materials sector, which includes miners, rose 0.8 percent as gold and copper extended their near week-long rally.

The most influential gainers included Eldorado Gold Corp, which rose 5.5 percent to C$11.10, and copper miner Teck Resources, up 2.8 percent to C$28.66.

Markets largely shrugged off U.S. data on Friday that showed America's economic growth slowed in the second quarter as consumers spent at their most sluggish pace in a year. While the figure was better than investors' worse fears it was still weak enough to potentially push the Fed closer to pumping more money into the economy.

'The economy would have to be much weaker than this for the Fed to really step in big time,' said Leitao, adding a smaller move such as extending the central bank's current ultra-low overnight interest rate to 2015 was more likely. 'That I think they can do and probably will do.'

Each year our neighbors spend their tax return on home improvements. Two years ago they refurbished their kitchen, last year they purchases new windows for the entire house and this summer they are giving their master bathroom a facelift. Given her passion for scrapbooking, it?s no surprise that each time my neighbor embarks on a redo the process ends up as the subject of yet another memory album.

Personally, I think scrapbooking home improvements is a phenomenal idea. Not only do you get to show off amazing before and after photos, but documenting the details of the demolition and construction is also beneficial for insurance purposes.

If you are looking for ways to create eye-popping page designs to showcase your home or room makeover, remember that the devil is in the details. Be sure to include even the smallest projects. Jonah ? A VeggieTales Movie #Review ? sensiblysara.com

My boys really enjoy movies. ?They aren?t terribly picky about what movie they watch, just as long as it comes from somewhere other than our movie shelves.

Because it?s summer, we?ve been letting the boys watch a movie after our bedtime routine. ?We?ve watched plenty of movies we own, and we?ve rented the rest. ?However, we are reaching the half-way point and they are ?out? of things to watch.

Mommy Bear Media to the rescue! ?I was really impressed by the very large selection of family friendly movies! ?It took awhile, but we finally decided on ?Jonah ? A Veggie Tales Movie.? ?The movie wasn?t as expensive as other places and it arrived very quickly! ?(Both things I appreciate, as I am sure you would as well!)

That weekend we decided to watch ?Jonah.? ?Typically, I enjoy Veggie Tales movies. ?They have a good message and they are enjoyable to watch. ?This one seemed slow. ?I had a hard time focusing on it. ?The music, however, was great!

I like that I can find movies that have been out awhile and recently released movies at Mommy Bear Media! ?If they are running low on stock, the page tells you how many are left. ?Each product page has a synopsis, rating, quality, UPC number, format, aspect ratio, language, run time, original broadcast date and so much more! ?I was really impressed by how much I could learn about the movie before deciding if I wanted that one of if I should move on to another selection.

(AP) ? A judge will consider Friday whether a police officer's moonlighting on a movie about his signature case warrants the dismissal of charges against a defendant who is still facing trial.

An attorney for Roy Lopez Jr., who is charged with burglarizing Paris Hilton's home, contends the officer's paid consultation on an upcoming Sofia Coppola movie about the case has irreparably tainted his testimony and credibility.

Los Angeles Police Officer Brett Goodkin interrogated many of the defendants and helped build the case against the so-called "Bling Ring" that stole more than $3 million in clothes, jewelry and art from the homes of stars such as Hilton, Lindsay Lohan, Orlando Bloom and Megan Fox and Brian Austin Green.

Records show Goodkin was issued $12,500 in checks for working as a consultant on Coppola's film "The Bling Ring," which stars Emma Watson and is currently being edited by the Oscar-winning writer-director. He also filmed a scene in which he arrests Watson's character, according to the Los Angeles Times, which first reported Goodkin's involvement in the film.

"To portray himself for the film and recreating his interaction with the defendants is extremely inappropriate and crushes the due process rights of the remaining defendants," Lopez's attorney David Diamond wrote in a motion seeking a dismissal of the case. The filing states Goodkin initially claimed he only received $6,000 for his work on the film, but the actual amount was more than double that figure.

Prosecutors contend Goodkin did not engage in conduct so outrageous that it violates Lopez's ability to receive a fair trial. Deputy District Attorney Christine Kee wrote in a filing this week that Goodkin did not commit a crime or encourage Lopez to commit one, two key factors that might warrant a dismissal if they occurred.

The motion will be heard by Superior Court Judge Larry P. Fidler, who at a previous hearing said there were parallels between the current case a California Supreme Court ruling in a case involving a prosecutor consulting on a film before trial. The case involved the murder trial of Jesse James Hollywood, whose case was the basis for the movie Alpha Dog." A Santa Barbara County prosecutor consulted on the film in an attempt to locate Hollywood, who was a fugitive at the time.

Hollywood's attorneys argued after his capture that the prosecutor's conduct required his removal from the case, but the state's high court disagreed and ruled that any issues could be resolved during jury questioning. The prosecutor, who was not paid for his work on the film, was later replaced on the case and Hollywood was convicted.

Diamond's motion notes that the only cases involving similar issues involve prosecutors, not police officers collaborating and working on films based on an active case.

It is the second time in the case that Goodkin's credibility has been questioned. Diana Tamayo, who is accused of breaking into Lohan's home, claimed she was coerced to confess after officers threatened her family with deportation and failed to properly administer a warning that she could remain silent. Fidler rejected a motion to dismiss charges against her.

Lopez, Tamayo and Courtney Leigh Ames have all pleaded not guilty to having any role in the break-ins.

Three other defendants have resolved their cases through plea deals and the group's alleged ringleader, Nicholas Prugo, has agreed to testify against Lopez and the remaining defendants.

Diamond said between Goodkin and Prugo, the prosecution is left with two witnesses who have severe credibility problems and that will benefit Lopez if the case proceeds to trial.

"There's not a shred of evidence against him and now we have the outrageous and egregious behavior of the officer assigned to supervise the case," Diamond said.

    One of the top communication tips for relationships is not to let a discussion deteriorate into an argument or a quarrel.

    For this to happen you have to avoid accusations. Don?t accuse; instead speak of the situation and what impact it had on you.

    2. Don?t sweat the small stuff

    Look at the bigger picture and don?t allow yourself to get hassled by the inconsequential stuff. It can be easy to get bogged down by minor details so don?t let those derail a discussion.

    3. Express your love frequently

    It?s no use saying that actions speak louder than words. Your significant other needs the words, needs to hear them often. The reassurance of the actual words can be important.

    4. Listen

    Really listening to the other person is one of the vitally important communication tips for relationships. Don?t just hear them out, listen and genuinely look at the other point of view and try to understand it.

    5. Be honest

    When it is your fault, fess up. Be honest and fair minded enough to acknowledge when you have been at fault. Do this for two reasons: firstly, it helps you come across as reasonable and just. Secondly, it prevents the escalation of the problem which could happen when the other person points out your mistake.

    6. Ask for help

    When you ask for help, you make the other person feel valued and wanted and important in your scheme of things. So asking for help is communication tips for relationships that shouldn?t be underestimated.

    7. If things get too heated, back off

    If a discussion becomes an argument and an argument deteriorates into a fight, this is not going to resolve the problem. If you see the discussion escalating, back off. Give yourself time to cool off before resuming the discussion.

    Leaving little notes for each other (or even texting) is one of the communication tips for relationships because it can be a fun and cute way to communicate. It reminds the other person of you and reminds them that you?re thinking of them when they are not around.

    When is being uncool cool? For Samsung, it?s when it gets you out of a potentially nasty legal battle with Apple.

    U.K. Judge Colin Briss ruled today that Samsung?s Galaxy Tab Android tablets don?t infringe on the iPad because they?re simply not as hip, Bloomberg reports.

    The Galaxy Tabs ?do not have the same understated and extreme simplicity which is possessed by the Apple design,? Birss said. ?They are not as cool.?

    More specifically, Birss said the Samsung slates were clearly different than the iPad because they were thinner and sported ?unusual details? on the back. Indeed, it?s hard to mistake an iPad for the Galaxy Tab if you have it in your hands, or can look at them from all angles.

    Last week, Apple won a preliminary injunction against Samsung over sales of the Galaxy Tab 10.1 in the U.S.

    (AP) ? IndyCar driver Justin Wilson blamed a faulty transmission for causing an accident in pit lane that sent two crew members to hospital during the Honda Indy Toronto's first practice session on Friday.

    Sebastien Bourdais' pit crew member Chuck Homan sustained the most severe injury after he had his leg pinned between two cars. Raffi Aroyan, also a member of Bourdais' Dragon Racing team, was also taken to hospital with an ankle injury.

    The crash happened in the pits, when Wilson had difficulty stopping his car and ran into the back of Bourdais' parked car.

    In a statement posted on his website, Wilson said he was stuck in first gear and couldn't get it into neutral. Compounding the problem, Wilson said, was the car's brakes were cold making it even more difficult for him to stop.

    "At the last second I tried to dive into the wall to take the speed off, but those guys were there and had no idea I was coming," Wilson said. "I really hope they are OK."

    Two others were hurt in the accident and were treated and released from the infield care center. They were Dragon Racing's David Martin and Michael Cole, a member of Wilson's Dale Coyne Racing team.

    Wilson returned to practice after the transmission problem was diagnosed, but his session ended quickly. Wilson was forced to put his car into the wall to avoid hitting Alex Tagliani, who had spun out in front of him in Turn 8.

    "It was just a horrible session," Wilson said. "We'll have to regroup and hopefully get back out for the next session."

    Wilson won at Texas last month and ranks 11th in the points standings.

    Qualifying in Toronto takes place Saturday. The race is Sunday.

    New Army radar could put drones in domestic airspace in 2014

    The U.S. Army announced that it has completed a two-week test of its Ground-Based Sense and Avoid system, which will allow drones and unmanned aircraft to fly more safely in domestic airspace. The system will allow for safer flights with less manpower and oversight, though it doesn't address the concerns citizens might still have regarding UAVs in their neighborhoods.

    Drones have been on the fast track at the FAA ever since their latest budget arrived with language suggesting they get moving on legalizing them. Police, national guard, scientists, everyone wants a drone, and while caution is a virtue at the FAA, sometimes things need to be expedited. Late?2015 is the hard limit on having regulations in place, and so the FAA is working with drone makers and operators to set down, for lack of a better term, some ground rules.

    Part of those rules is a "sense and avoid" system, with which an unmanned aircraft can detect other craft in the same airspace and make the appropriate adjustments to its flight path. Today, that often means a chase aircraft with a human observer, but low visibility or darkness can put an end to that. Ideally, the craft itself would house instruments capable of scanning the area, but ground-based systems can be more comprehensive and more reliable, though they are less portable.

    The GBSAA is a 3-D radar system that tracks every craft in range,?calculates the likelihood of conflict, and suggests a course correction if necessary. The Army tested it in a series of "vignettes," in which real aircraft were used to invade the drone's operational space, and they say the new system passed with flying colors.

    Viva Austin, product director for the Army's Unmanned Systems Airspace Integration program, described the difficulty of producing anything like a threat to the system:

    Air traffic control keeps people separated so well, it was kind of hard to put yourself in a really stressing situation and test those algorithms out really well. It was very safe and we demonstrated that the system and the test bed was really successful.

    The only reservation they had with the tests was that the GBSAA would suggest flight paths that would not normally be taken by pilots ? for instance, it might pick a safe?route that veers north, while a pilot would veer south because it's more towards base, or away from restricted airspace.

    If things go well, drones could be flying above a number of US forts as early as March of 2012. This doesn't mean they'll be hovering outside your window, but it is a step in that direction. Privacy and reasonable use by the authorities will be a completely different set of decisions and legislation, though; first they have to be air-legal, then they can be put to use.

    Assad friend flees, U.S. wants Russia to "pay price"

    PARIS (Reuters) - As a Syrian general who was a personal friend of Bashar al-Assad fled for France, Hillary Clinton urged Assad's enemies meeting in Paris on Friday to make Russia and China "pay a price" for helping keep him power in Damascus.

    The Secretary of State's remark, among Washington's toughest yet in 16 months of revolt, highlighted the gulf between Western and Arab countries, who met opposition groups in Paris to try to engineer Assad's departure, and his two supporters on the other side of the old Cold War divide, who stayed away from the talks.

    "I will tell you very frankly," Clinton told the Friends of the Syrian People conference, "I don't think Russia and China believe they are paying any price at all - nothing at all - for standing up on behalf of the Assad regime. The only way that will change is if every nation represented here directly and urgently makes it clear that Russia and China will pay a price because they are holding up progress - blockading it.

    "That is no longer tolerable."

    The foreign ministers and senior diplomats from 50 Western, Arab and other countries - including Assad's former ally and neighbor turned bitter foe Turkey - urged stronger U.N. Security Council action and "broader and tougher" sanctions.

    They also agreed to "massively increase" aid to Syria's rebels and to provide them with communications equipment, according to a final statement. Western powers are reluctant to offer much greater firepower to rebels whose ranks include anti-Western Islamists. Divisions among Assad's foes, visible notably in a fistfight at an opposition meeting in Cairo this week, have also limited the coordination of efforts to remove him by force.

    As Clinton spoke, Manaf Tlas, a brigadier in the Republican Guard who attended military college with Assad and whose father was Assad's father's confidant and defense minister for 30 years, was on his way to Paris, the French government said.

    There was no immediate sign that Tlas, who friends said had fled Damascus for Turkey this week, would throw in his lot with the rebels. He did not attend Friday's meeting.

    But his defection was the clearest signal yet that some in Assad's inner circle think his days in power are numbered, as an uprising that began in March 2011 with a groundswell of peaceful protest turns into a civil war with strong sectarian overtones.

    Tlas's father Mustapha now lives in Paris, as does Tlas's sister. The family has been a rare representative of Syria's Sunni Muslim majority in an elite dominated by Assad's fellow Alawites. Sunnis have been in the forefront of the uprising.

    DESERTION "MEANS NOTHING"

    Assad's enemies among Syrians and in the West were quick to hail Tlas's flight as a breakthrough, while Damascus played it down. A Syrian news website quoted a Syrian official on Thursday as saying: "His desertion means nothing ... If Syrian intelligence had wanted to arrest him it would have."

    Opposition sources have spoken of senior figures who are under suspicion of being potential defectors being held under virtual house arrest. Assad's opponents have been trying to encourage as many of his entourage as possible to abandon him.

    While the lightly armed rebels are no match for Syria's large and well-equipped army, their hope lies in eroding loyalty and conviction within Assad's establishment to the point where it loses its hold on power.

    Syria's army took control of the rebel stronghold of Khan Sheikhoun in northern Idlib province on Friday after an assault on the town backed by helicopters, rebels said.

    "The Free (Syrian) Army withdrew from the town last night after it ran out of ammunition. Assad's army is in control," said Abu Hamam, a rebel spokesman who fled to a nearby village.

    "They are burning the houses. They have burned my own house. I see the smoke covering the sky from where I am now."

    Army shelling and assaults also killed three people in the southern province of Deraa, where the nationwide revolt began.

    Opposition activists say more than 15,000 people have been killed in the uprising, while the government says several thousand members of the security forces have died.

    The host of the Paris meeting, French President Francois Hollande, said Assad's departure was "in the interest of Syria, of its neighbors and everybody who wants peace in the region".

    Clinton repeated a U.S. and French call for a U.N. Security Council resolution under Chapter 7 of the U.N. Charter, which allows the Council to authorize actions ranging from diplomatic and economic sanctions to military intervention:

    "We should go back and ask for a resolution in the Security Council that imposes real and immediate consequences for noncompliance, including sanctions under Chapter 7," she said.

    Western governments have shown no appetite for military intervention like the NATO bombing that helped oust Libya's Muammar Gaddafi last year, and U.S. officials have repeatedly said in the case of Syria they are talking only about sanctions.

    Russia and China have in the past vetoed U.N. Security Council resolutions designed to pressure Assad, and say they are committed to a peace plan drafted by international envoy Kofi Annan that proposes dialogue without calling for Assad to go.

    PLAN STILLBORN

    That plan has been stillborn because the ceasefire that was supposed to precede it in April never took hold. Peace monitors effectively gave up on their mission last month after just weeks in Syria as it became clear there was no peace to monitor.

    With heavy fighting now on the outskirts of the capital, events on the ground are outrunning diplomatic initiatives.

    A senior French diplomat said recent rebel territorial gains had led to signs that even Moscow was now envisaging a post-Assad Syria, something Russian officials strongly deny.

    "The situation on the ground has changed fast over the past three weeks, with security forces having no access to some areas," the French diplomat said. "We are now hearing things from within political and military circles in Russia that are surprising us and that we were not hearing before."

    Tlas's defection may indicate a growing alienation among the Sunni business elite, which had been slow to embrace the revolt which began among poorer parts of the majority community.

    Opposition activists said Tlas would announce that he had abandoned Assad because of anger at civilian deaths. A witness in Damascus said by phone that Tlas's house in Damascus had been ransacked by security agents after reports that he had fled.

    French Foreign Minister Laurent Fabius said the defection meant that Assad's inner circle "is beginning to understand that the regime is unsustainable".

    Foreign Minister Ahmet Davutoglu of Turkey, Syria's NATO-member neighbor, agreed: "Every day, generals, colonels, officers are coming, and we have, I think, around 20 generals and maybe 100 high-ranking officers, colonels," he told France 24 television.

    Turkey has moved artillery and troops towards its border with Syria in the two weeks since Syria shot down a Turkish warplane at the frontier. Turkey now says it will treat Syrian units that approach the border as hostile.

    Saudi Arabia and Qatar want to fund and arm the rebels, but Western powers have misgivings about sending more weapons into what could become a wider sectarian conflict.

    Qatari Prime Minister Sheikh Hamad bin Jassim al-Thani called in Paris for the Security Council, where Russia and China have the power to veto any action, to be sidestepped.

    "Are we able to do something outside the Security Council?" he asked. "Yes. Have we done things outside the Security Council in the past? Yes, and there are many examples.

    "We are ready to take part in any effort of any kind to free the Syrian people of this tragedy they are in."

    While Syria's rebels enjoy wide popular sympathy among fellow Arabs, the support of the Sunni-ruled, pro-Western Gulf monarchies also reflects their strategic goal of thwarting the regional ambitions of Assad's sponsors in Shi'ite-ruled Iran.

    Hi guys,

    Was thrilled to find this forum which is so active & with varied interests.

    Wanted to ask which other forums are there which are India centric & active.

    I have come across many forums but they tend to be US or Europe specific & are not relevant in Indian context.

    I am looking for forums on health, parenting, home improvement, community, day to day living, city entertainment life, movies, shopping and anything else you guys frequently visit.

    Please share links & your comments on it.

    (AP) ? When Michelle Wie's name comes up, many fans likely remember a child prodigy expected to dominate women's golf for years.

    Today, Wie is a 22-year-old recent Stanford graduate. And in most of the tournaments she has played this year, her primary concern hasn't been contending ? but whether she'd simply make the cut.

    That trend turned around in a big way Friday, when Wie shot a 6-under 66 at Blackwolf Run and climbed to within a shot of the lead in the U.S. Women's Open.

    "I'm pretty stoked to be back in contention and honestly not have to worry about the cut line," Wie said. "It feels pretty good."

    Suzann Pettersen, the Norwegian star ranked sixth in the world, shot a 68 and moved to 5 under to take the lead going into Saturday's third round. Wie was one stroke back, tied with 2007 Open winner Cristie Kerr.

    Wie said she doesn't spend much time thinking about the attention she received when she was younger, or whether some fans had written her off since then.

    "I don't know if anyone gave up on me or not," Wie said. "I'm sure some did and some didn't. But I never gave up on myself, and today was a good reminder to myself that I can do (it) and I still have it."

    Wie is thankful for the opportunities she had, but isn't particularly eager to revisit them.

    "I can't really live in the past," Wie said. "What I did is what I did, and I'm really looking forward to what I'm going to do tomorrow and Sunday and the future, really."

    On Friday, she got it done with accurate approach shots ? then making putts.

    "Yesterday I had a lot of 40-foot putts, 50-foot putts," Wie said. "Today on the back 9 I had a lot of putts within 15 feet. That really helps on the golf course. It's the difference between lagging them and trying to make them. So I was putting them in good positions today."

    Wie was 12 years old when she qualified for an LPGA Tour event in 2002 ? making her at the time the youngest player ever to do so. She went on to compete in a handful of men's tournaments, but never turned her early stardom into consistent success. She has two career LPGA Tour victories.

    Wie has missed six cuts this season, although she says she has been feeling better about her game.

    "The last couple of weeks it started to feel good," Wie said. "Things were coming. My scores weren't showing up as well. But it's a work in progress. Still a long way to go."

    And Wie knows she's a long way from a potential victory Sunday, but is excited to have something to shoot for beyond simply making it to the weekend.

    "The fact that you're in contention to have a chance to win the U.S. Open is a big deal," Wie said. "I'm so grateful I have that chance right now. I'm really looking forward to seeing the crowds tomorrow and experiencing it all again."

    Pettersen said Wie should be given some space to find her game at her own pace.

    "I think you should give her a break," Pettersen said. "She just graduated, four years in college. That's pretty impressive to do that on the sideline of trying to compete out here. So now it's obviously a little different world for her. Now it's all about golf, and she has to kind of find her schedule, how to kind of work it out the best way for her."

    Pettersen's own day got off to a frantic start when her alarm clock betrayed her. She woke up late and had to rush to the course.

    Talk about a golf scramble.

    "I thought it was awfully light in the room when I woke up at 6:40, an hour late," Pettersen said. "I don't know. I looked at the alarm clock, and I think instead of putting it (at) 5:45, I put it for 6:45. I cut breakfast short and went straight to the green."

    And after another day in hot and muggy conditions, Pettersen said, "Can't wait for a shower now."

    Sandra Gal, Inbee Park and Vicky Hurst shot 70 to reach 3 under.

    Lizette Salas, a 22-year-old recent Southern California graduate, had a 73 to drop to 2 under. Mika Miyazato also was 2 under after a 71.

    Brittany Lincicome had a miserable day, shooting an 80 on Friday to fall to 5 over.

    "I did nothing right today," Lincicome said. "I couldn't drive the ball. I couldn't do anything right. I couldn't putt."

    Seventeen-year-old Lexi Thompson shot a 73 to top the group at 1 under.

    Top-ranked Yani Tseng was 2 over after a 72.

    Second-ranked Stacy Lewis recovered from a first-round 77 to shoot a 69 on Friday, putting her 2 over. Third-ranked Ai Miyazato, the winner last week in Arkansas, was even par after a 74.

    Defending champion So Yeon Ryu shot a 71 and was 1 over.

    Se Ri Pak, the Open winner at Blackwolf Run in 1998, also was 1 over after a 73.
